These are the following rates for out of pocket payment or before out of network reimbursement:
60 min Initial Diagnostic Evaluation : $350 (CPT code 90792)
30 min follow up $200 (CPT code 99214)
60 min follow up $310 (CPT code 99215+90838)

- Dr. DePalma is an “out of network Physician” and does not currently contract with any insurance plans. (Why no Insurance contracts)
*Many of our patients have been able to get part or all of fees paid by insurance as "out of network". Please ask them if this is important to you (see below)
- An invoice with appropriate diagnostic information will be provided so patients may submit for reimbursement to insurance company by sending to the address on the back of your card.
- Fees are due in full on arrival for appointment.
In other words on or before the time of appointment you pay for the appointment. At the end of the appointment you get an invoice that you can send to your insurance company.
