Medication Management

When medication is appropriate, Dr. DePalma’s depth of training in psychopharmacology means prescribing decisions are precise, individualized, and grounded in a thorough understanding of each patient’s biology and clinical history. The right medication, used correctly, can be a powerful tool. The wrong medication — or medication used as a substitute for real clinical assessment — can cause harm. Dr. DePalma takes this seriously.

Psychotherapy

Cognitive Behavioral Therapy (CBT) is a highly targeted, well-researched approach that works directly with dysfunctional patterns of thought and behavior. The stream of daily thinking — much of which we accept without examination — often generates unnecessary suffering. CBT makes that stream visible and workable. New neural pathways are built through this process; harmful ones are allowed to atrophy.

Acceptance and Commitment Therapy (ACT) / Mindfulness – deceptively simple yet deeply effective approaches. It removes much of the elusiveness from mindfulness practice by giving patients a clear, structured framework for engaging with difficult thoughts and emotions without being controlled by them. Problems, emotional wounds, and suffering are approached with clarity and without judgment. ACT can facilitate lasting behavioral and pattern change when practiced with consistency.

Psychodynamic Psychotherapy at its best is a nuanced and powerful process. Emotion becomes a map — a guide to fundamental insight, growth, and freedom from longstanding pain and maladaptive patterns. The first sessions focus on understanding the issues at hand. From there, Dr. DePalma and the patient together choose the approach most suited to that individual’s situation and goals.

Ketamine Treatments

Ketamine therapy has emerged as one of the most significant advances in psychiatric treatment in decades, with a growing and substantial body of evidence supporting its efficacy in treatment-resistant depression, anxiety, PTSD, and other conditions where conventional approaches have fallen short. Unlike traditional antidepressants that work gradually over weeks, ketamine produces rapid neurobiological changes that can shift a patient’s trajectory meaningfully and sometimes dramatically.

Dr. DePalma most often administers ketamine within a protocol of Ketamine-Assisted Psychotherapy — an integrated approach that goes considerably beyond ketamine infusion alone. This protocol is grounded in an understanding of what ketamine actually does at the neurobiological level: it increases neuroplastic potential, activates hypoactive brain regions, alters dysfunctional network activity, and enhances communication between brain networks that depression and trauma have left isolated or dysregulated. The result is a window — a period of heightened neurological flexibility during which the brain is measurably more receptive to change than it ordinarily is.

Dr. DePalma uses that window deliberately. Psychological therapy is integrated into the ketamine protocol to guide and process the experience, help the patient work through longstanding emotional patterns, and anchor the neurobiological shifts ketamine produces into lasting psychological change. The combination is not incidental — it is the point. Ketamine alone opens the door. Skilled psychotherapy, delivered by the same clinician who understands both the pharmacology and the patient’s full clinical history, walks through it.

This is what separates the One Psychiatry approach to ketamine from clinics that administer infusions in isolation. The neuroplastic window ketamine creates is a clinical opportunity. How that opportunity is used determines the depth and durability of the outcome.

About One Psychiatry

One Psychiatry is a private-pay psychiatric practice serving adults in Scottsdale and Phoenix, Arizona. The practice was founded on the belief that effective psychiatric care requires time, clinical depth, and a genuine relationship between patient and physician — not the hurried, transactional model that has become common in insurance-driven psychiatry.

Dr. DePalma is both the prescribing physician and the treating therapist for his patients. This integrated approach means medication decisions and psychotherapeutic work inform each other continuously, producing more coherent and effective care than the fragmented model of seeing separate providers for each.

New patients are seen by appointment. Dr. DePalma is currently accepting new adult patients in Scottsdale and Phoenix.
